About Our Practice
James River Family Practice is honored to manage your healthcare needs.
Our services include care of chronic medical conditions, urgent and preventative care, physicals, DOT physicals, pediatrics (age two and older), minor surgery, gynecology, geriatrics, and work related injuries. Dr. Halverson and Dr. Beavers admit patients and conduct rounds daily at Riverside Regional Medical Center.
Your Initial Visit
Please complete all patient information forms prior to your first appointment. These forms can be downloaded and printed from the new patient registration tab. Please arrive fifteen minutes prior to your appointment so that we can review your information and update your chart. Please bring all of your medications and your insurance card(s) with you. If possible, please bring your medical records with you or the name and address of your previous physician. Any information regarding your past medical history would be helpful. Please be advised that we do not accept personal checks at your first visit. Cash, credit cards (VISA, Master Card and Discover), as well as money orders are accepted.
An appointment is needed in order to be seen. Please call us in advance so that a special time may be reserved for you. If you need to be seen and have not scheduled an appointment, please call our office to find out how we may accommodate you. Emergencies will be dealt with immediately. If you are unable to keep your scheduled appointment, call us at least 24 hours prior to reschedule. A $30.00 fee is charged for appointments that are not cancelled 24 hours in advance. Also, there is a fee to complete forms that are not done during an office visit. This fee can range from $15.00 to $30.00 depending on the type of form. There is also a $10.00 fee for any letter requests.
If you are under 18 years old, you need to be accompanied by a parent or guardian or have a note from them giving us permission to see you.
Please arrive 15 minutes before your scheduled appointment time to complete registration materials, update your insurance records, and pay any required co-pays. We will then update your health records and medication list and prepare you for your visit with the doctor. Please know the name, dosage, and how often you take your medication. We suggest that you write all of this down before you come in. Be sure to include all nonprescription medications, such as laxatives, herbal supplements, vitamins, etc.
Our office will be closed every 2nd Friday of every month for staff training from 12:30-2:00.
Payment is required at the time of the visit. If we participate with your insurance company, we will collect your co-payment or deductible at the time of the visit. We accept cash, checks, Master Card, VISA and Discover.
We participate with most major health insurance and health maintenance organization (HMO) programs.
• Please bring your insurance card with you for each visit.
• If you are unsure about coverage, please call our office to confirm our participation. As a courtesy, we file all insurance claims.
• We recommend you become familiar with your health insurance policy; its coverage, payments, limitations, and regulations concerning referrals and pre-authorization. Because all insurance contracts are between the patient and the insurance company, you are ultimately responsible for payment.
Your condition will determine if you require care outside of our office, whether it involves a specialty referral, emergency treatment, hospitalization, or diagnostic testing. Unless you have an emergency, we prefer that you contact our office before seeking care elsewhere.
We will assist you in obtaining the medical care you need. If necessary, we will refer you to a specialist. Please allow a minimum of two working days to accomplish this.
Please call and inform your pharmacist that you need a refill. Prescription refills should be requested 48 hours in advance and during office hours only. Be prepared to provide the prescription number and other information on the label. Ask the pharmacist to fax the request to the correct office location.
Prescriptions for antibiotics, pain medicines, or other non-routine medications require an office visit so that a review of your chart and an examination may be completed to better determine the appropriate course of care and proper medications.
Medical Records and Release of Information
We maintain strict confidentiality of the information contained in your medical record. To assure this confidentiality, information contained in your medical record will not be released to anyone without your written permission as directed by HIPAA law.
We review every lab and diagnostic test returned to the office and you should be notified of the results. Please contact our office if you have not received notification within one week of completion of your test. We encourage all patients to sign up for our portal so that you will have direct access to your test results.
